ABSTRACT

BACKGROUND@#Acute type A aortic dissection (ATAAD) and acute type A intramural hematoma (ATAIMH) are life-threatening diseases with high mortality. To better understand their clinical features in the Chinese population, we analyzed the data from the first Registry of Aortic Dissection in China (Sino-RAD) to promote the understanding and management of the diseases.@*METHODS@#All patients with ATAAD and ATAIMH enrolled in Sino-RAD from January 1, 2012 to December 31, 2016 were involved. The data of patients' selection, history, symptoms, management, outcomes, and postoperation complications were analyzed in the study. The continuous variables were compared using the Student's t test for normal distributions and the Mann-Whitney U test for non-normal distributions. Categorical variables were compared using the Chi-square test or Fisher exact test.@*RESULTS@#A total of 1582 patients with ATAAD and 130 patients with ATAIMH were included. The mean age of all patients was 48.4 years. Patients with ATAAD were significantly younger than patients with ATAIMH (48.9 years vs. 55.6 years, P < 0.001). For the total cohort, males were dominant, but the male ratio of patients with ATAAD was significantly higher compared to those with ATAIMH (P = 0.01). The time range from the onset of symptom to hospitalization was 2.0 days. More patients of ATAIMH had hypertension than that of ATAAD (82.3% vs. 67.6%, P < 0.05). Chest and back pain were the most common clinical symptoms. Computerized tomography (CT) was the most common initial diagnostic imaging modality. 84.7% received surgical treatment and in-hospital mortality was 5.3%. Patients with ATAAD mainly received surgical treatment (89.6%), while most patients with ATAIMH received medical treatment (39.2%) or endovascular repair (35.4%).@*CONCLUSIONS@#Our study suggests that doctors should comprehensively use clinical examination and genetic background screening for patients with ATAAD and ATAIMH and further shorten the time range from symptoms onset to intervention, achieving early diagnosis and treatment, thereby reducing the mortality rate of patients with aortic dissection in China. We should standardize the procedures of aortic dissection treatment and improve people's understanding. Meanwhile, the curing and transferring efficiency should also be improved.

Objective: Securidaca inappendiculata is a medicinal plant frequently used in the treatment of inflammatory diseases in south China. In this study, we aimed to explore its bioactive constituent which contributes to the anti-inflammatory activity. Methods: Polyphenol-enriched and polyphenol-deprived fractions (PRF and PDF, respectively) were separated from the ethanolic extract by HPD300 macroporous resin-based method, and their anti-inflammatory activities were investigated on a lipopolysaccharide (LPS)-induced acute lung injury (ALI) model in rats. ABSTRACT

Aim To investigate the gene frequency of UGT1 A1?6 in cancer patients in Anhui Han popula-tion. Methods The 222 cases of blood samples of Han cancer patients were collected from different re-gions of Anhui province, and the UGT1A1?6 geno-types were detected by in situ hybridization fluorescencestaining. Results Patients with a UGT1A1?6 wild type ( GG ) accounted for ( 159 cases, 71. 62%) , which were higher than those of heterozygous mutations ( GA, 52 cases, 23. 42%) and of homozygous muta-tions ( AA, 11 cases, 4. 96%) of the total cases. The mutation rate of UGT1 A1?6 was 16. 67%, and partic-ularly in patients with esophageal cancer it was 43. 75%. The rates of mutation in the patients in Ma ' anshan and Chuzhou were 40. 01% and 34. 62%, re-spectively, both significantly higher than those of othertumors and regions. Conclusions Cancer patients in Anhui Han population have a high mutant frequency of UGT1 A1?6 . The UGT1 A1?6 genotyping can indi-rectly predict the risk of irinotecan's adverse reaction, which obviously enhances the potentially individualized treatment of irinotecan.

ABSTRACT

Twenty phenylpropenamide analogs with structural novelty were designed and synthesized upon pharmacophore-combination strategy. The structures of target compounds were elucidated by IR, 1H NMR, 13C NMR and MS, and all the target compounds were biologically evaluated for the inhibitory activities of platelet aggregation induced by adenosine diphoshate (ADP) and (AA) arachidonic acid via Bron method. As a result, compounds 6b, 9b, 9d and 9h demonstrated potent inhibitory activity against platelet aggregation induced by AA. Meanwhile, compounds 6b, 6d, 6j, 9b and 9g exhibited significant suppression of platelet aggregation induced by ADP.

ABSTRACT

Objective Cerebral small vessel disease is closely related to kidney disease .Chronic kidney disease ( CKD) may increase the risk of hemorrhage stroke .However, its impact on hemorrhage-prone small vessel disease represented by cerebral microb-leeds( CMBs) remains unclear .The purpose of this study was to investigate the relationship of CKD with the presence and location of CMBs in patients with acute lacunar stroke . Method Consecutive patients with acute lacunar stroke within 7 days from onset were enrolled retrospectively from January 2014 to July 2016 and scanned by gradient-echo T2*-weighted imaging (GRE-T2*WI).Their demographic, clinical, laboratory and imaging data were collected .Estimate glomerular filtration rate (eGFR) was calculated individu-ally by the following chronic kidney disease epidemiology collaboration (CKD-EPI) equation for the Asian population .CKD was defined as the level of eGFR<60 mL/min/1.73 m2. Results Finally, 308 patients (mean age:65.79±8.67 years; median NHISS:3(2-5);42.2%Female) with lacunar ischemic stroke were enrolled in the final analysis .Among these patients, CMBs were present in 116 patients ( 37.7%) and CKD in 62 patients ( 20.1%) .Patients were divided into CKD group and normal group according to GFR level . The result of univariate analysis showed that patients with CKD had higher prevalence of diabetes ( P=0.014) and higher degrees of CMBs (P=0.001) compared with normal group.CMBs were refined by its location .The result of multivariable analysis showed that CMBs in deep brain [ OR=7.61, 95%CI 4.18-16.55, P=0.001] were sig-nificantly associated with CKD incidence , while no significant rela-tionship was found in CKD incidence and CMBs in the lobe and mixed location of brain . Conclusion The CKD incidence in patients with acute lacunar stroke is in dependent relationship with CMBs in deep brain and without significant correlation with CMBs in the lobe and mixed location of brain .

ABSTRACT

Six novel ligustrazine chalcone aromatic oxygen alkyl acids compounds and two pyridine chalcone aromatic oxygen alkyl acids ester compounds were synthesized according to the traditional Chinese medicine theory removing blood stasis. The structures of target compounds were identified by IR, NMR and ESI-MS. The inhibitory activities of platelet aggregation induced by adenosine diphosphate (ADP) and arachidonic acid (AA) were measured by the liver microsomal incubation method in vitro. Hypolipidemic activities of compounds were tested in vivo for better inhibitory activities of platelet aggregation. Preliminary pharmacological results showed that compounds 7c, 8a and 11a had potent inhibitory activity against platelet aggregation induced by AA, compounds 7c, 7d, 8a and 11b showed significant inhibitory activity against platelet aggregation induced by ADP. Compounds 7c and 8a exhibited good hypolipidemic activities in high-fat-diet (HFD) induced hyperlipidemia C57/BL6 mice and worthy for further investigation.

OBJECTIVE@#To discuss the role of intestinal flora imbalance in the pathogenesis of pouchitis.@*METHODS@#The pouchitis rat model was established and the faeces sample and the mucous membrane sample were collected regularly, in which the bacterial nucleic acids were extracted for quantitative analysis of the intestinal flora in the samples through using the real-time quantitative PCR technique and high energy sequencing technology.@*RESULTS@#The disorder phenomenon of the intestinal flora appeared at the 7th day of the experiment, and the pouchitis was presented at the 21st day of the experiment. At the 31st day of the experiment, compared to control group and non-pouchitis group, the quantity of Bifidobacterium and the Lactobacillus of the pouchitis model rats in the mucous membrane sample and the faeces sample were significantly decreased (P < 0.05), and the Bacteroidetes, Faecalibacterium prausnitzii and XIV Clostridium leptum subgroup in the mucous membrane of pouchitis were significantly decreased (P < 0.05). The IV Clostridium coccoides group was the main flora in the mucous membrane of pouchitis, the bacterial diversity of non-pouchitis group and control group was significantly higher than that of the pouchitis group (P < 0.05).@*CONCLUSIONS@#The intestinal flora imbalance is one of the factors that cause the incidence of the pouchitis; this study provides a clue of the pathogenesis and treatment direction of the intestinal inflammatory disease.

AIM@#To study the chemical constituents from the fermentation of the endophytic fungus HP-1 of Chinese eaglewood.@*METHODS@#The chemical constituents were isolated by column chromatography on silica gel and Sephadex LH-20, and their structures were elucidated on the basis of spectroscopic analysis.@*RESULTS@#Four compounds were isolated and identified as 3α, 3β, 10β-trimethyl-decahydroazuleno[6, 7]furan-8, 9, 14-triol (1), 4-hydroxyphenylacetic acid (2), 4-hydroxyphenethyl alcohol (3), and 5-hydroxymethyl-2-furancarboxaldehyde (4).@*CONCLUSION@#Compound 1 was a new compound. Compound 2 showed antibacterial activity against Staphylococcus aureus.

AIM@#To investigate the chemical constituents in the stems of Trigonostemon heterophyllus.@*METHOD@#The chemical constituents were isolated by column chromatography on silica gel, Rp-18, and Sephadex LH-20, and their structures were elucidated on the basis of spectroscopic analysis.@*RESULTS@#Three compounds were isolated and identified as a new diterpene, trigonoheterene B (1), together with two known compounds, trigonostemone (2) and trigonochinene B (3).@*CONCLUSION@#Compound 1 is new. Compounds 2 and 3 showed antibacterial activities.

ABSTRACT

Objective To study any protection against hippocampal neuron damage induced by epilepsy (SE) provided by transcutaneous stimulation (TNS) of the trigeminal nerve and to document any effect of such stimulation on the expression of glutamic acid decarboxylase (GAD) 65/67.Methods Pilocarpine injection was used to induce epilepsy in healthy male Sprague-Dawley rats which were then randomly divided into a treatment group and a model group.Rats which had not received the pilocarpine injection served as normal controls.In the treatment group the rats were given electrostimulation for one month after the first spontaneous seizure following the injection of pilocarpine.In the model group they were given sham TNS for one month.After the month of stimula-tion,immunohistochemistry was used to detect the expression of GAD65/67 in the hippocampus.Terminal deoxy-nucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) assays and Nissl staining were applied to deter-mine apoptosis and neuron loss in the hippocampus.Results Significantly less apoptosis was observed in the treatment group than in model group at 24 h,48 h and 72 h post-injection.Compared to the model group,average GAD65/67 expression had increased significantly in the treatment group at 24 h,72 h,1 week,2 weeks and 4 weeks post-stimulation.GAD65 expression reached its peak from 72 h to 1 week post-stimulation,then decreased to the level of the control group by 4 weeks post-stimulation.The expression of GAD67 remained elevated at all the time points employed.Conclusions TNS can significantly protect hippocampal neurons from damage in epilepsy,at least in rats.The underlying anti-epileptic and neuroprotective mechanisms may involve increased inhibitory transmission induced by the stimulation.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of ethyl acetate fraction of Blumea balsamifera (BBE) residue on treating rats of adjuvant arthritis (AA) and its mechanism.</p><p><b>METHOD</b>The rats were immunized with the Freund's complete adjuvant (FCA). After modeling, 28 days' treatment with BBE was performed. During the experimental process, rat mass, toe girth, arthritic index (AI), proliferation of immune organs and pathological section were measured. After treatment, blood samples were collected through fossa orbitalis vein for detection of serum SOD, MDA, GSH, NO, OH*, ALP, AST, ALT, NAG and SA content using colorimetric method and IL-1, IL-6, TNF-α content using ELISA method.</p><p><b>RESULT</b>Administration with BBE (high dose) could significantly ameliorate joint swelling and arthritis index, effectively inhibit synovial hyperplasia, down-regulate the levels of MDA, NO, OH*, ALP, AST, ALT, NAG, SA, IL-1, IL-6, TNF-α and up-regulate the SOD and GSH levels in serum.</p><p><b>CONCLUSION</b>The results suggested BBE possesses substantial anti-arthritis and antioxidant activities.</p>

ABSTRACT

Objective To observe any influence of hyperbaric oxygen (HBO) treatment on the formation of glial scars,and to explore how HBO suppresses the inflammatory reaction to injury.Methods A total of 96 healthy,adult,male,Sprague-Dawley rats were used to model cerebral puncture injury.They were then randomized into a control group and a treatment group,with 48 rats in each group.The treatment group received HBO treatment,while the control group received no special treatment.At 1,3,7,14 and 28 days after the puncture injury,the rats' right brain tissues were harvested and immunohistochemical staining was employed to compare the changes in number of astrocytes and microglial cells around the injury in the two groups.The level of tumor necrosis factor α (TNF-α) and interleukin 1 β (IL-1β) in the cerebral tissue was examined using ELISA.Results Among the control group the average wound areas after 7,14 and 28 days were (2.73 ± 0.05)μm2,(3.42 ± 0.18)μm2 and (2.41 ± 0.09) μm2,a significant reduction after 28 days compared with 7 and 14 days.The corresponding average wound areas of rats in the treatment group were (2.78±0.12)μm2,(2.59 ±0.08)μm2 and (1.20 ±0.06)μm2.There the average wound area had decreased significantly after 14 days,and the further reduction after 28 days was also significant.The numbers of GFAP-positive astrocytes at 14 and 28 days had increased significantly compared with after 7 days in both the control group and the treatment group.The average number of GFAP-positive astrocytes in the control group at 28 days had decreased significantly compared with after 14 days.Compared with the control group at the same time points,the number of GFAP-positivc astrocytes in the treatment group was significantly less.After modeling,the number of ionized calcium-binding adapter molecule Ⅰ (Ibal)-positive microglial cells increased significantly,but there was a significant decrease in both the control and treatment groups by 7 days.The average number of Ibal-positive microglial cells in the treatment group was significantly less than in the control group at all of the time points.Compared with the first day after modeling,the TNF-α concentration of the controls at 3 and 7 days was significantly higher,but by the 7th day it was significantly lower than it had been after 3 days.The average IL-1β concentration in the control group and TNF-α concentration in the treatment group had increased by day 3,but then decreased by day 7.The IL-1β concentration of the treatment group declined gradually.The average TNF-α and IL-1 β concentrations of the treatment group were significantly lower than those of the control group at all of the time points.Conclusion HBO treatment has a relatively good curative effect on cerebral puncture injury.It can accelerate wound healing and reduce the formation of glial scars.Its mechanism could be related to the deactivation of astrocytes and microglia cells and reducing the levels of cell factors that promote inflammation.

ABSTRACT

To investigate the chemical constituents of the endophytic fungus Penicillium sp. FJ-1 of Ceriops tagal, the chemical constituents were isolated by column chromatography on silica gel and Sephadex LH-20. Their structures were elucidated on the basis of spectroscopic analysis. Their antibacterial activity was tested by paper disco diffusion method. Two compounds were isolated and identified as 7-hydroxy-deoxytalaroflavone (1), and deoxytalaroflavone (2). Compound 1 is a new compound, and compounds 1 and 2 showed weak activity against Staphylococcus aureus and methicillin-resistant Staphylococcus aureus.

<p><b>OBJECTIVE</b>To study the constituents from the stems of Aquilaria sinensis.</p><p><b>METHOD</b>The chemical constituents were isolated by various column chromatographic methods. The structures were identified by spectral analysis including NMR and MS data.</p><p><b>RESULT</b>Sixteen compounds were isolated and identified as threo-buddlenol C (1), thero-ficusesquilignan A (2), erythro-buddlenol C (3), (+/-) buddlenol D (4), (-) medioresinol (5), (-) pinoresinol (6), 5'-methoxy lariciresinol (7), erythro-guaiacylglycerol-beta-coniferyl ether (8), thero-guaiacylglycerol-beta-coniferyl ether (9), herpetin (10), (+) syringaresinol (11), curuilignan (12), ciwujiatone (13), coniferyl alcohol (14), 3, 4, 5-trimethoxyphenol (15) and cucurbitacin (16).</p><p><b>CONCLUSION</b>All the compounds, except for 11-13 were obtained from A. sinensis for the first time.</p>

ABSTRACT

Objective: To investigate the chemical constituents in the leaves of Ilex kudingcha. Methods: Repeated column chromatography with silical gel, maroporous resin column, Sephadex LH-20, and preparative HPLC were used to isolate the chemical constituents, and their structures were elucidated on the basis of spectral data. Results: Eight compounds were isolated and identified as mateglycoside D (1), methyl caffeate (2), vanillin (3), gallic acid (4), caffeic acid (5), β-sitosterol (6), p-hydroxy-benzaldehyde (7), and ferulic acid (8). Conclusion: Compounds 2 and 7 are first obtained from the plants of Ilex L., and compounds 1,3,4, and 8 are first obtained from I. kudingcha.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the ultrastructural variation of the facial nerve of rabbit with different dosage of (125)I seed brachytherapy.</p><p><b>METHODS</b>Fifty-four big ear rabbits were divided into 3 groups randomly and given 40 Gy, 80 Gy, 120 Gy respectively. Radioactive seeds were implanted in one side of parotid gland, the other side was implanted with vacant shell as a control group. The facial nerves were obtained 2, 4, 6 months respectively after operation and the histological ultrastructural changes observed by electromicroscope.</p><p><b>RESULTS</b>In the control group, epineurium was continuous, there was slight pitting edema under the epineurium, and axonal myelin was loose. In the test groups, there was slight pitting edema under the epineurium, and axonal myelin sheath was loose at 4th month. Macrophage and regenerated fibers were found in the 80 Gy group and myelin sheath lamellar separation, regeneration of nerve in the 120 Gy dosage. The myelin sheath lamellar was separated and axonal myelin loose in the test group at 6th month. Myelin sheath amellar separation and edema under the epineurium were found in the group of 80 Gy and 120 Gy.</p><p><b>CONCLUSIONS</b>The ultrastructure of the facial nerve is damaged by the dosage of 40 Gy, 80 Gy brachytherapy with (125)I seeds. The higher dosage the nerve receives, the more serious the damage will be. Both of the epineurium and axonal myelin sheath are integral and continuous 6 months after operation with dosage of 120 Gy.</p>

<p><b>OBJECTIVE</b>To evaluate the safety and efficacy of staged hybrid approach in treating ventricular septal defect (VSD) patients combined with patent ductus arteriosus (PDA) and pulmonary artery hypertension (PAH).</p><p><b>METHODS</b>From July 2004 to July 2009, 22 VSD patients with PDA and PAH were enrolled and received staged hybrid approach treatment (transcatheter PDA occlusion and elective open surgery for VSD several days after PDA occlusion). All patients were followed up to examine rhythm change, residual shunt, shape of occlude, possible valve regurgitation, and aortic stenosis by echocardiography.</p><p><b>RESULTS</b>After transcatheter PDA occlusion, pulmonary arterial systolic pressure decreased from (76.2 ± 25.8) mm Hg (1 mm Hg = 0.133 kPa) to (55.4 ± 20.6) mm Hg (P = 0.005), mean pulmonary artery pressure decreased from (53.5 ± 23.5) mm Hg to (36.2 ± 17.8) mm Hg (P = 0.049), total pulmonary resistance decreased from (8.2 ± 4.9) wood units to (6.9 ± 4.3) wood units (P = 0.037), and pulmonary-to-systemic flow ratio (Qp/Qs) increased from 2.8 ± 2.3 to 3.4 ± 1.7 (P = 0.045) post transcatheter interventional PDA occlusion. After VSD repair, pulmonary arterial systolic pressure decreased from (64.5 ± 22.3) mm Hg to (43.1 ± 18.9) mm Hg (P = 0.001) and mean pulmonary artery pressure decreased from (40.2 ± 18.7) mm Hg to (29.5 ± 15.8) mm Hg (P = 0.040). There was no death or right heart failure during the follow-up.</p><p><b>CONCLUSION</b>Staged hybrid approach is an effective and safe strategy for treating VSD patients with PDA and PAH.</p>

ABSTRACT

Objective To evaluate the feasibility and effect of transthoracic echocardiography(TTE)on guiding the occlusion of the soft-rim atrial septal defect(ASD).Methods Sixty two patients with the soft-rim ASD were enrolled.The size of ASD was measured and rim of ASD was observed by TTE on various views by using color Doppler system with tissue harmonic function before occlusion,and filmy rim of ASD with flapping which could not sustain occluder was eliminated.The size of occluder was selected by integratively judging the size of ASD and"sustainable diameter of ASD"The waist size of occluder was measured after releasing occluder and compared with the longest diameter of ASD and"sustainable diameter of ASD"measured by TTE.Results The longest diameter of ASD measured by TTE before occlusion was 11-35 mm[average(21.6±5.2)mm],the "sustainable diameter of ASD"was 15-37 mm[average(25.6±5.(J)mm],the size of selected occluder was 18-44mm[average(30.7±5.5)mini and the waist size of released occluder was 13-35 mm[average(24.2±5.6)mm].Fine correlation was existed between the longest diameter of ASD measured by TTE and the waist size of released occluder(r=0.86,P<0.000I).Morever,improved correlation was found between the"sustainable diameter of ASD"measured by TTE and the waist size of released occluder(r=0.89,P<0.0001).Occluder was firmly fixed without falling in all patients.Conclusions TTE with tissue harmonic function can be used to measure the size of soft-rim ASD and the"sustainable diameter of ASD".It is a feasible,and effective method on guiding occlusion of soft-rim ASD.

ABSTRACT

OBJECTIVE: To explore the antitumor activities of kushen (Sophora flavescens) flavonoids (KS-Fs) in vivo and in vitro. METHODS: Cell proliferation was assayed by using methyl thiazolyl tetrazolium (MTT) method. H22 hepatocellular carcinoma and S180 sarcoma were induced in ICR mice. Lewis lung carcinoma was induced in C57BL/6 mice. H460 and Eca-109 tumor were induced in Balb/c nude mice by injecting 5x10(5) or 5x10(6) tumor cells in the right flank, respectively. RESULTS: KS-Fs could inhibit the growth of a variety of human tumor cell lines (A549, SPC-A-1, NCI-H460, etc.) in vitro. The antitumor efficacies were confirmed in the mice models of H22, S180 and Lewis lung tumors and the nude mice models of human H460 and Eca-109 xenografted tumors. The oral or intravenous maximum tolerated dose of KS-Fs was more than 2.8 g/kg or 750 mg/kg respectively, far more than the oral medial lethal dose of kushen alkaloids (< or = 1.18 g/kg). No adverse reactions were observed. CONCLUSION: These results suggest that KS-Fs or kurarinone may be developed as a novel antitumor agent.
